*JOB SUMMARY*
This full time, at will, position is responsible for providing day-to-day support to the Lead Receptionist and a team of Aging and Disability Resource Center (ADRC) Options Counselors and other assigned roles.
This leader provides support and guidance, motivating the team to meet the group’s responsibilities with the highest quality of service.
*ESSENTIAL DUTIES*include the following.
Other duties may be assigned and/or requested from time to time in fulfillment of the agency’s mission.
1.
Provides leadership to the team and works with members to build unity and cohesiveness 2.
Assists with the hiring process to fill vacant positions in team 3.
Coordinates and contributes to the training process for new staff members 4.
Offers all team members training, coaching, re-training, and discipline as appropriate 5.
Inspires person-centered care philosophy 6.
Assures the agency Personnel Policies and Code of Ethics are followed 7.
Ensures program timelines and guidelines are met 8.
Ensures team responsibilities are met, performing staff duties when temporary shortages of staff occur 9.
Serves as a liaison between staff, A2AA administration, State staff, individuals, families, and other community stakeholders, while providing outstanding customer service 10.
Manages administrative responsibilities for at least two ADRC Options Counselors, including but not limited to, managing and approving time cards, time off requests, expense sheets, staff evaluations, benchmarks, and reports 11.
Monitors quality outcomes of the team, communicates and assists in building team understanding and compliance with expectations for outcomes 12.
Communicates information to the team, passing on information in a timely manner while retaining the integrity of the message 13.
Serves individuals in a manner that is empowering, patient, compassionate, and person-centered 14.
Attends, participates, and leads meetings and trainings 15.
Contributes to the functioning of the ADRC for all team members by answering calls and assisting walk-in guests 16.
Maintains the team’s workflow 17.
Performs and/or coordinates community presentations and representation at resource fairs 18.
Manages project for the ongoing maintenance of resource data stored in Vision Link *RESPONSIBILITIES* This positions makes a substantial contribution to the mission of REAL Services, Inc.
due to the fact that this person provides the leadership to build and maintain a solid and well-functioning team as the Agency’s representatives to the community.
The attitude and culture of a team is directly influenced by the qualities of this individual.
In addition, the quality of care delivered to clients is dependent on whether this individual has helped to cultivate and inspire an effective, organized, and coordinated team.
This leader serves a group of direct reports and the ADRC team members as a whole.
*EDUCATION AND/OR EXPERIENCE REQUIREMENTS* Bachelor’s degree (BA or BS) from an accredited four-year college; or, an associate’s degree may be considered with four or more years’ experience in human services, assessment, service plan development and implementation.
Obtain and maintain ongoing Care Manager Certification with the Indiana FSSA Division of Aging.

Ability and attitude to work productively, independently, cooperatively, and relate to the needs of individuals, colleagues, the agency, and community partners 6.
Ability to follow instructions and procedures, to interpret the information and to complete assigned tasks in an accurate, concise, and detailed manner 7.
Knowledge of in-home and community based services, funding sources, eligibility, community resources, and information available through REAL Services and in the community 8.
Ability to assess specific needs both in-person and on the phone 9.
Ability to develop rapport quickly, and to empathize and understand individuals’ behaviors 10.
Ability to use state mandated computer programs, resource database, as well as basic knowledge of Excel and Word computer programs, as relevant to successfully execute the job 11.
Ability to educate team members and community *DIFFICULTY OF WORK* Guidelines are generally clear and complete, with some judgment required in implementing them when unusual circumstances arise.
The work demands sound judgment be exercised in selecting the most tactful approach in communicating with team members and with individuals served.
All documentation must be accurate and timely.
The work is repetitive in nature inasmuch as the same tools, forms, and procedures are utilized routinely, but does involve variables and considerations when dealing with diverse situations.
